Hep C Link to Care is an educational and screening initiative directed at healthcare providers involved with identifying and treating patients infected with the Hepatitis C virus (HCV).
 
The goal of the program is to:
  •  Provide physicians with the most current education and information on Hepatitis C
  •  Conduct HCV screening programs in at-risk communities
  •  Establish coalitions of healthcare providers to ensure clear links to follow-up care for all
   persons who test positive for HCV
The program includes live educational meetings, screening events in locations where persons may be at risk for HCV infection, and web-based resource directories for linking patients with HCV to quality care.

Round Approximately 1 out of every 50 of your patients has chronic viral hepatitis.
Round An estimated 75-80% of persons with chronic hepatitis C were born between 1945 and 1965.
