| Hep C Link to Care is an
educational and screening initiative directed at
healthcare providers involved with identifying
and treating patients infected with the
Hepatitis C virus (HCV). |

| The goal of the program is to: |
| |
• Provide physicians with the most
current education and information on Hepatitis C |
|
| |
• Conduct HCV screening programs in
at-risk communities |
|
| |
• Establish coalitions of healthcare
providers to ensure clear links to follow-up
care for all persons who test positive for HCV |
|
| The program includes live
educational meetings, screening events in
locations where persons may be at risk for HCV
infection, and web-based resource directories
for linking patients with HCV to quality care. |

Approximately 1 out of every 50 of your patients has
chronic viral hepatitis. |
|
 |
An estimated 75-80% of persons with chronic
hepatitis C were born between 1945 and 1965. |
|

Hepatitis C can be cured, yet most patients don't
know they are infected and less than 12% are on
treatment. |
